How to obtain a proof of vaccination without a DigiD

If you want to obtain a proof of vaccination but you do not have a DigiD, you can contact the CoronaCheck Helpdesk to request for a COVID Certificate on paper. For information on converting your non-EU vaccination certificate, visit our FAQ page.

 

How to apply

For everyone living in the Netherlands with a citizen service number (BSN)

If you live in the Netherlands and have been vaccinated with a European Medicines Agency (EMA) approved vaccine or a vaccine on the Emergency Use Listing of the World Health Organisation (WHO), you can apply for a COVID certificate by calling the CoronaCheck helpdesk at 0800-1421 or email helpdesk@coronacheck.nl

For Privileged Persons or diplomatic status holders

Privileged Persons who have been vaccinated by the GGD but are not in possession of a DigiD can send a request to GGD Haaglanden via email for a COVID Certificate on paper. Please include the following information in your application:

  • First name(s)
  • Last name
  • Date of birth
  • Postal code + house number
  • City
  • Location and dates of vaccinations
  • Address where the COVID Certificate can be sent to securely
  • Phone number

The application must be accompanied by:

  • A copy of the diplomatic passport or diplomatic identity card (issued by the Dutch Ministry of Foreign Affairs)
  • A copy of the vaccination registration card (note: a copy of the “Yellow Book” will not be accepted).

In special cases, the applicant may be asked to identify himself/herself – by appointment – at the office of the GGD Haaglanden.
